I agree that all information I provide regarding my animal’s behaviour, history, and any relevant circumstances is truthful, accurate, and complete to the best of my knowledge.
I acknowledge that withholding, misrepresenting, or failing to disclose important details—such as previous aggression, medical conditions, or behavioural concerns—may pose a significant risk to the safety of the practitioner, myself, my animal, and others.
I understand that under the Animal Welfare Act 2006, I have a legal duty of care to ensure the welfare of my animal and to prevent harm to others. I am also aware that, in the case of dogs, the Dangerous Dogs Act 1991 and related legislation require me to prevent my dog from being dangerously out of control.
I accept that failure to provide full and honest disclosure may result in the immediate termination of services without refund. I understand that Caroline Clark of Pet Education and Training will not be held liable for any consequences arising from such omissions, and I take full responsibility for any legal repercussions that may result from undisclosed behavioural issues or other relevant factors.
